Lifespan of Greater Rochester Wins Prestigious Award

Archstone Foundation Award 2019

Lifespan of Greater Rochester recently won the Archstone Foundation Award for its Community Care Connections (CCC) program. In 2015, Lifespan created the CCC pilot program with support from the New York State Department of Health. The goal was to prove that integrating traditional community-based partnerships with healthcare providers positively affects cost, quality, and patient and physician satisfaction. The program has received national recognition.
 
Lifespan contracted with Rochester RHIO to provide encounter data for pre- and post-intervention comparisons for the CCC program. The New York Academy of Medicine (NYAM) then used the data to evaluate CCC's effectiveness and return on investment (ROI). Overall, the program achieved several positive outcomes. Estimated ROI was $4 per dollar spent on providing care through the program. Read more about the award.
